imagine360 Powered by Quantum Health is willing to work with any provider, but not every provider is willing to work with imagine360. Please review the Provider Search and reach out to a Care Coordinator with any questions or if you need assistance finding a new provider.

What to do if your Provider isn't listed or you are receiving push back

  • Connect with a Care Coordinator by phone, chat or message. They are able to reach out to the provider or facility to educate them on the plan and answer any questions. If your provider is unwilling to work with the plan, they can also assist you with finding a new provider.

Quality Score

Health Care Blue Book™ evaluates, scores, and compares quality among providers and facilities.

Network Affiliations (AKA Provider Level)

Tier 1: Imagine Health

  • Existing facility relationship
  • Reduced deductible for additional savings

Tier 2: Partners Direct Health and Open Access

  • Partners Direct Health has an existing provider relationship
  • Open Access has accepted claims from the provider in the past but work with a Care Coordinator on any provider questions

Historic Provider Acceptance

  • Shows prior experience with provider's acceptance
  • 3 rating means they historically accept
  • Ratings do not guarantee future acceptance – call your Care Coordinator

Cost Estimate

  • Will give a summary of the cost and your estimated total out-of-pocket for certain procedures

Each Estimate Provides:

  1. Procedure information, including the cost before the member’s benefits are applied.
  2. Your out-of-pocket procedure cost, including your co-insurance (if applicable).
  3. Information about your benefits plan.
  4. Total accumulated toward your annual deductibles and out-of-pocket maximum.
  5. Ability to text, email or print a cost estimate.
