Medical Plan
Keep your most important machine running its best! Having medical insurance you can count on is important in maintaining your health, preventing illness, and getting you through when you aren't feeling your best.
Plan Options
Businessolver offers 2 High Deductible Health Plans (HDHPs) to choose from: Summit 3500 and Horizon 2500. Both plans offer a different level of benefits and comes with a different premium cost. These plans will be available from both imagine360 and Aetna (4 plans in total). Be sure to compare your options and choose the plan that works best for your care needs and the price you prefer to pay.
Preventive Care
Your health plan includes preventive care, which is a critical step you can take to manage your health. For a list of preventive services covered by your health plan, contact your Quantum Care Coordinator at the number on your Benefits ID card.
Price Protection
Price protection helps to to lower your out-of-pocket costs by reviewing medical claims to make sure you only pay what is fair and reasonable. While some provider may receive a payment lower than what they billed, most accept the plan’s payment.
If you are ever billed for more than the out-of-pocket responsibility that is listed on your Explanation of Benefits, or if you have a question about a bill, call the Quantum Health Care Coordinators right away at the number on your Benefits ID card.
Deductibles
Your deductible follows the calendar year and resets on January 1 each year. If you change Businessolver medical plans mid-year, credit for the deductible you have met will be applied to the new plan.
Medical Premium Bands
Organizations across the U.S. are experiencing increased health care spend. Businessolver is not exempt, yet we are focused on the costs of Solvers' out-of-pocket contributions. 
With that in mind we have adopted a salary banded model for premiums. Here is the breakdown:
- Less than $60,000 Annually
- $60,000 - $89,999 Annually
- $90,000 - $119,999 Annually
- $120,000+ Annually
On average, Businessolver contributes 80% of the monthly premium. Below are the 2025-2026 rates per pay period for both medical plans. In the event of months with three pay periods, you will not see premiums deducted for the third paycheck of those months.

Who is Eligible?
You (and, if applicable, your spouse, domestic partner, and/or dependent children) are eligible for Businessolver's insurance coverage if:
- You are an active full-time Businessolver employee
- You work at least 30 hours per week as a part time Businessolver employee
- If your spouse/domestic partner has access to other medical insurance coverage, they are not eligible to be covered under Businessolver's medical plan.
- An eligible child is married or unmarried and is under 26 years of age. Coverage will end at the end of the month in which they turn 26.
Additional Information
Prescription Coverage - Rx coverage is carved out of the medical plan and is offered through Optum Rx. This does not require a separate enrollment, and your Rx cost still apply to the medical deductible and out-of-pocket maximum.
