MEDICAL
DLL offers three medical plans (PPO, EPO, HDHP w/ HSA) with the ability to cover members, spouses/domestic partners, and dependent children
Employee Costs (bi-weekly)
Members are eligible to receive a wellness credit for next year’s (2023) medical election, if all 2022 requirements are met. The annual wellness credit is $500 for the member and an additional $200 if your spouse completes the wellness rewards criteria.

Health Savings Account (HSA)
If you are enrolled in the HDHP medical plan and are eligible for the HSA, DLL will fund your account based on your election.
DLL will contribute to the HSA accounts for new hires enrolling in the HDHP plan on a prorated basis. This contribution will be made on a biweekly frequency. You will have the opportunity to contribute to the account with pre-tax elections throughout the year. DENTAL
The dental vendor is UCCI for 2022.

Vision
The vision vendor is Eyemed for 2022. There will be no plan or contribution changes.

Life Insurance
DLL provides members with employer-paid life and accidental death and dismemberment insurance which provides you with 1.5 times your compensation up to $600K. Members have the option to elect supplemental life insurance for yourself, your spouse/domestic partner or your dependent children. Evidence of insurability may be required.
Disability Insurance
DLL pays Basic Short-Term and Long Term Disability.
The Basic Short-Term Disability benefit begins after a five-day elimination period, you may receive up to 25 weeks of compensation at 100% or 60% of earnings depending on your length of service.
The Basic Long-Term Disability (LTD) benefit provides eligible members a benefit of 60% of annual earnings to a maximum of $12,000 per month.
