Additional Enhanced Benefits
You have access to enhanced benefits at no cost to you and your dependents enrolled in an Envision medical plan:
- Hinge Health virtual physical therapy (newly added).
- 2nd.MD expert second opinions and coverage for second opinions through 2nd.MD was expanded (newly expanded to cover more family members).
- Lantern personalized guidance for non-emergency surgeries.
- Health Advocate support for understanding your coverage, resolving claims and assisting with billing issues.

Flexible Spending Account (FSA) Contribution Limit Increase
Dependent Care FSA limit is increasing to $7,500 for single individuals or married couples filing taxes jointly and $3,750 for married couples filing separately.
Healthcare FSA is increasing to $3,400
Healthcare Costs: What to Expect in 2026
Healthcare Costs: Across the U.S., medical costs continue to rise each year, driven by increased utilization and higher costs of new medical technologies, hospital services and prescription drugs. Nationally, medical costs are expected to increase by 8–9% for 2026.
How Our Plan Works: At Envision, our medical plan is self-insured. This means that the insurance carrier processes claims, but the actual dollars for doctor visits, prescriptions, surgeries and hospital stays are paid by Envision and our teammates together. Envision continues to cover the majority of these costs; however, our year-over-year medical claims have increased by 12.5%, which is significantly higher than the national average.
2026 Medical Premiums: To help address these rising costs, medical premiums for 2026 will increase by 9% for all benefit-eligible teammates.
Envision is absorbing a larger share of the rising expenses so that teammate premium increases remain below our actual medical cost growth. These adjustments are necessary to help protect the long-term sustainability of our medical plan and preserve the quality and breadth of coverage available to teammates and their families.
Envision uses salary bands to determine medical premiums. If you had a change in compensation that moves you into a new salary band, you will experience additional changes to your medical premiums.
2026 Deductible, Coinsurance and Out-of-Pocket Maximum Changes: Labs and X-rays will be subject to plan deductible and coinsurance under the Point of Service (POS) Select and Primary plans.
Medical plan deductibles and out-of-pocket maximums will increase in 2026. This is the result of Envision’s increasing cost of healthcare and IRS requirements that govern High Deductible Health Plans.

Eligibility
- You are eligible for the benefit program if you work at least 117 hours per month or 27 hours per week.
- Your dependents may participate in your benefit plans if they meet all eligibility requirements.
- Envision will conduct eligibility audits for both you and your dependents to ensure compliance.
- For current teammates making benefit changes during Annual Enrollment, your coverage is effective January 1 .
- For newly eligible teammates, your coverage is effective the first day of eligible employment, provided you enroll within 30 days from your date of hire or date of eligibility.
Paying for Benefits
Benefit Deductions
Teammates who are paid bi-weekly (i.e., 26 times per year) will only have regular benefit deductions from 24 pay periods each calendar year. Premiums shown during your enrollment will be based on 24 pay periods. This means you will get two "benefit-free" paychecks each year!
Missed Deduction Reminder: On our benefits website, you will find your personalized rates for each available benefit program. You are responsible for your portion of premiums and any assessed surcharges as of your benefits effective date. If there are missed deductions, they will be automatically collected on future paychecks. This may result in larger reductions of your take-home pay until all missed deductions have been collected. Examples include leaves of absence and missed or incorrect deductions.

Visit the benefits website via Okta, or log in to EnvisionHealthcareBenefits.com.
If you access the site directly and need to reset your password, click Forgot your user name or password? under the Login button.

With Your Mobile Device
The MyChoice® benefits app puts your benefits website in the palm of your hands. If you don't already have the app, visit our benefits website via Okta or by logging into EnvisionHealthcareBenefits.com, click Access the App from the home page, and follow the prompts to download the app directly to your mobile device.
MyChoice Benefits App
The MyChoice benefits app lets you upload verification documents, store pictures of your ID cards, access coverage details, and change your benefits via Okta or by logging into EnvisionHealthcareBenefits.com. Simply go to the homepage of the benefits website and click Access the App to download the app and get started.
