Verifying Your Dependents
If you choose to enroll your spouse and/or child(ren), you will be prompted to complete a dependent reverification during the enrollment process. All team members are required to reverify enrolled dependents, even if documentation has been previously submitted. Be sure to have your most recent tax return (Form 1040) available to upload when prompted for the quickest and easiest way to verify all dependents with one document.
Designate Your Beneficiaries
Beneficiary information is required for your Life Insurance, Health Savings Account (HSA)/Flexible Spending Account (FSA), and/or 401(k), if elected.
To add or update your Life Insurance beneficiary, visit InspireBrandsBenefits.com and select Change My Benefits > Basic Info.
To add or update your HSA, FSA, and/or 401(k) beneficiary information, visit Netbenefits.fidelity.com and go to Profile > Beneficiaries. Each Fidelity account requires a separate designation.
