Dependent Reverification


As part of the 2026 enrollment process, you will be required to verify any dependents that you enroll, even if you have previously provided documentation. This process helps ensure that our benefits remain affordable and sustainable for you and your family, and that benefits are only extended to those who qualify.

Important: You will need to submit the required documentation by November 30, 2025. If you do not provide the necessary documentation, your unverified dependents will be dropped from coverage on December 31, 2025.

How to Submit Dependent Documents

You'll see an alert under your My Tasks on the Home page of InspireBrandsBenefits.com. From there, follow the prompts to upload required documentation.

Tip: PDFs or JPEGs work best for uploading. Make sure your uploaded document is clear, legible, and all required information is available.

Below is a list of the acceptable forms of documentation that can be used to verify your dependent(s).

Spouse

Most recent IRS 1040 / federal tax return (page 1) that shows dependent's name and relationship

If married less than 1 year, a Government-issued Marriage Certificate will be acceptable.

Child

Most recent Tax Return

--OR--

Government-issued Birth Certificate naming team member as parent

--OR--

Hospital documentation reflecting the child’s birth, naming team member as parent (acceptable if child is under 6 months of age)

Stepchild

Requirements for Verification of Spouse

--OR--

Child’s Government-issued birth certificate naming team member's Spouse as the child’s parent

Adopted Child / Child of Legal Guardianship

Most recent Tax Return

--OR--

Adoption Decree / Court documentation confirming adoption

--OR--

Court documentation confirming Legal Guardianship

Disabled / Incapacitated Child
Most Recent federal tax return showing the name of the employee or verified spouse, and the child's government-issued birth certificate naming you as the parent --OR--

Government-issued birth certificate naming you as the parent

--OR--

Hospital documentation reflecting the child's birth, naming you as parent (if under 6 months of age ONLY)

AND

**In addition to the verification requirements

Social Security Administration income statement

--OR--

Disabled dependent documentation that shows total disability/incapacity prior to age 26 to medical plan deductible

Providing false, misleading, or incomplete information will be subject to disciplinary action, up to and including termination.
