Dependent Verification Audit


We are conducting a Dependent Verification Audit this year. This audit is a standard process to confirm all dependents enrolled in our benefits plans meet eligibility requirements. This process helps ensure that our benefits remain sustainable for you and your family, and are only extended to those who qualify.

Important: You will need to submit the required documentation by November 30, 2025. If you do not provide the necessary documentation, your unverified dependents will be dropped from coverage on December 31, 2025.

How to Submit Dependent Documents

You'll see an alert under your Important Reminders on the Home page of https://www.inspirebrandsbenefits.com/. From there, follow the prompts to upload required documentation.

Tip: PDFs or JPEGs work best for uploading. Make sure your uploaded document is clear, legible, and all required information is available.

Most recent IRS 1040 Tax Return (first page only) indicating married and filing jointly or separately

If married less than 1 year, a Government-issued Marriage Certificate will be acceptable.

Child

Government-issued Birth Certificate naming member as parent

Hospital documentation reflecting the child’s birth, naming member as parent (acceptable if child is under 6 months of age)

Stepchild

Requirements for Verification of Spouse

Adopted Child / Child of Legal Guardianship

Adoption Decree / Court documentation confirming adoption

Most Recent federal tax return showing the name of the employee or verified spouse, and the child government-issued birth certificate naming you as the parent OR

Government-issued birth certificate naming you as the parent

OR

Hospital documentation reflecting the child's birth, naming you as parent (if under 6 months of age ONLY)

AND

Social Security Administration income statement

OR

Disabled dependent documentation that shows total disability/incapacity prior to age 26dition to medical plan deductible
