About Connected Care Providence
Providence is the largest health system in the Portland-metro area, with a comprehensive, high-value network of highly respected providers, facilities and services. Providence clinicians use one medical record to provide a seamless care experience. This means wherever you receive care from a Providence provider, every caregiver can access up-to-date information about your health through the same electronic file.
Connected Care Providence is designed to provide you and your family a higher-quality, better-coordinated and more collaborative health care experience that places you at the center.
Connected Care Providence HDHP
This option is a High Deductible Health Plan (HDHP) designed to help you and your family take control of your healthcare dollars and decisions by providing you with the flexibility and control in choosing the healthcare services you and your family members receive and how to pay for the cost of these services. There is no annual paycheck deduction, however there is an annual deductible that must be paid before the HDHP pays benefits. Once the deductible is met, you will pay a 5% coinsurance for in-network care. You can offset your out-of-pocket cost by making pre-tax contributions to a Health Savings Account and use those funds to pay for eligible health care expenses.
Prescription Drug Coverage: Prescription drugs are included with Connected Care HDHP. Prescriptions have a 5% copay after deductible. Certain medications are covered 100%. The Formulary Drug List is a list of covered brand-name and generic medications that your medical option has identified to be efficacious, safe, and cost-effective. It is periodically updated. To review the list of covered medications, along with the cost, go to www.providence.org/locations/or/intel.
Connected Care Providence Primary Care Plus
The Primary Care Plus option requires an annual paycheck deduction but has a lower annual deductible and copays for some services like office visits and prescriptions. You don’t need to meet the deductible before the option pays for in-network primary care office visits and prescription drugs.
Prescription Drug Coverage: Prescription drugs are included with the Connected Care Primary Care Plus. For a 30-day supply you will pay a $10 copay for generic, a $20 copay for formulary and $35 for non-formulary. The Formulary Drug List is a list of covered brand-name and generic medications that your medical option has identified to be efficacious, safe, and cost-effective. It is periodically updated. Anthem Blue Cross PCP
The Anthem Blue Cross Primary Care Plus (PCP) provides choice on when and how you pay for healthcare to our employees nationwide. It is administered by Anthem and uses the same Anthem doctors as the Anthem Blue Cross High Deductible Health Plan (HDHP).
You pay a paycheck contribution for the Anthem Blue Cross PCP, and it provides:
- 100% coverage for in-network preventive care
- Low copayments for primary care and specialty care office visits and prescription drugs
- A deductible with coinsurance for all other care (e.g., laboratory services, hospitalization, etc.)
With a low copayment for primary care, you are encouraged to build a strong relationship with your primary care provider. A fixed copayment for prescriptions provides you the comfort of knowing what your ongoing monthly prescription expenses will be. If you need other in-network care, you have assurance knowing the out-of-pocket maximum amount protects you from undue financial hardship.
Prescription Drug Coverage: Prescription drugs are included in the Anthem Blue Cross PCP and have a $10 copy for generics, a $20 copay for formulary drugs and a $35 copay for non-formulary drugs. The Formulary Drug List is a list of covered brand-name and generic medications that your medical option has identified to be efficacious, safe and cost-effective. It is periodically updated.
You may fill up to a 30-day supply of your maintenance medications at the network pharmacy of your choice. A 90-day supply is available at Walgreens, Costco and ESI mail pharmacy.
