Limited Medical Coverage Options
These plans are offered by VAULT Limited Medical Plans and are underwritten by Nationwide Insurance Company of Columbus, Ohio. If you are seeking an alternative to COBRA and the full coverage medical plan options, then you may want to consider a limited coverage plan. These plans provide less coverage than the marketplace plans and COBRA but will exceed the minimal essential coverage requirement under the ACA and likely have lower monthly costs. A benefit to these plans is that you can enroll year-round at an affordable monthly price. MEC Visit
MEC Visit offers full coverage for covered preventive health services. It also provides a copay arrangement for many in-network providers. Formulary-restricted pharmacy benefits are also covered, depending on the type and tier of the drugs. Urgent care, surgeries, emergency room visits and other hospital stays are not covered. There is no deductible, and no out of pocket maximum.
Elite Health
The Elite Health Plan provides full coverage for covered preventive health services. It also provides a copay arrangement for many in-network providers. Out-of-network or hospital-provided services are not covered with the Elite Health Plan. Formulary-restricted pharmacy benefits are also covered, depending on the type and tier of the drugs. There is no out-of-pocket maximum.
Elite Health Plus
Elite Health Plus offers full coverage for covered preventive health services. It also provides a copay arrangement for many in-network providers, including limited in-network hospitalization. It also provides a copay arrangement for office visits and other medical services (in-network and not at a hospital). Formulary-restricted pharmacy benefits are also covered, depending on the type and tier of the drugs. There is a $1,000/$2,000 (Individual/Family) deductible to be met before coinsurance/copay coverage begins. There is no out of pocket maximum.
Elite MEC (Previously EASE Primary - Grandfathered in)
Elite MEC is the lowest coverage level in our minimum coverage options. It offers coverage for preventive and wellness services, telemedicine consults and some prescription subsidies. Members would pay out of pocket for office visits and hospitalization. There is no deductible and no out of pocket maximum.
Premium Health
Premium Health offers full coverage for covered preventive health services. It also provides a copay arrangement for many in-network providers, including urgent care. Formulary-restricted pharmacy benefits are also covered, depending on the type and tier of the drugs. Surgeries, emergency room visits and other hospital stays are not covered. There is no deductible and no out-of-pocket maximum.
