Important Documents Needed When Adding Dependents

If you complete a life event (such as marriage, divorce, adoption), you will need to submit supporting documentation to the Benefits Department for review.

This year, we are conducting a Dependent Verification Audit starting on September 3, 2025. It's a standard process to confirm all dependents enrolled in our benefits plans meet eligibility requirements. This helps ensure that our benefits remain sustainable for you and your family, and are only extended to those who qualifying.

In the meantime, start gathering your documents now so you are ready on September 3, 2025.

Below is a chart that outlines the forms of documents needed:

Domestic Partner Affidavit (located in the Reference Center) - AND - 2 forms of Proof of Joint Obligation dated in the last 12 months (examples below)
Children Under Age 26

Proof of Joint Obligation Examples

If you need to submit forms that show Proof of Joint Obligation, here are some examples that you can use:

  • Mortgage statement
  • Bank statement
  • Joint utility bill
  • Monthly bill from a rental/lease agreement
  • Joint ownership of a motor vehicle
  • Listed as a beneficiary on life insurance
