Eligibility
- You are eligible for the benefits program if you work at least 117 hours per month or 27 hours per week.
- Your dependents may participate in your benefits plans if they meet all eligibility requirements.
- Envision will conduct eligibility audits for both you and your dependents to ensure compliance.
- For newly eligible clinicians and teammates, your coverage is effective the first day of eligible employment, provided you enroll within 30 days from your date of hire or date of eligibility.

Paying for Benefits
Clinicians and teammates paid bi‑weekly (26 pay periods per year) will see benefit deductions taken from 24 paychecks annually. Premiums displayed during enrollment are based on these 24 pay periods. This means you’ll receive two benefit‑free paychecks each year.
Your personalized benefit rates, including premiums and any applicable surcharges, are available on the benefits website. You are responsible for your portion of costs beginning as of your benefits effective date.
If a deduction is missed, it will be automatically collected through future paychecks. This may result in temporarily higher deductions until the balance is resolved. Missed deductions can occur for reasons such as leaves of absence or incorrect deduction.
Medical plan premiums are determined using Envision’s salary band structure, helping ensure contributions are aligned and consistent.
