You can also contact the Mondelēz Canada Benefits Centre at 1-800-641-1500 (Option 1 – Benefits; Option 1 – Health & Welfare Benefits) if you have questions. VERIFY YOUR DEPENDENTS
You’re responsible for ensuring that each individual you enrol for an Optional coverage meets the Flexible Benefits Program’s eligibility requirements at all times.
If your child is turning age 19 (or 21 if in Quebec) and is a full-time student at an accredited institution, you’ll need to certify their student status via MyBenefits Online. A month before your child’s birthday, an “Over Age Dependent” certification event will be queued up on MyBenefits Online for you to complete. If you don’t take action, the event will default, and your child’s coverage will terminate. If you need help confirming your child’s student status and/or to reinstate coverage, call the Mondelēz Canada Benefits Centre at 1-800-641-1500 (Option 1 – Benefits; Option 1 – Health & Welfare Benefits).
Any misrepresentations or inaccurate information you provide could result in loss of coverage. And, if you cover an individual who isn’t eligible, GreenShield reserves the right to recover all amounts that result from an overpaid or unsupported claim. If this is the case, GreenShield will deduct such amounts from future claims and/or take action by other legal means.
