Limited Coverage Medical Plans

In 2021 there will be five limited coverage medical options. Learn more about each option below:

EASE Primary (Previously WellMEC)

EASE Primary is the lowest coverage level in our minimum coverage options. It offers coverage for preventive and wellness services, telemedicine consults and some prescription subsidies. Members would pay out of pocket for office visits and hospitalization. There is no deductible and no out of pocket maximum.

WellPremium (Previously WellPrime)

The WellPremium Plan provides full coverage for covered preventive health services. It also provides a copay arrangement for many in-network providers. Out-of-network or hospital-provided services are not covered with the WellPremium Plan. Formulary-restricted pharmacy benefits are also covered, depending on the type and tier of the drugs. There is an out-of-pocket maximum.

WellPlus 20MP

WellPLUS 20MP offers full coverage for covered preventive health services. It also provides a copay arrangement for many in-network providers, including limited in-network hospitalization. It also provides a copay arrangement for office visits and other medical services (in-network and not at a hospital). Formulary-restricted pharmacy benefits are also covered, depending on the type and tier of the drugs. There is a $1,000/$2,000 (Individual/Family) deductible to be met before coinsurance/copay coverage begins. The Plan features an out-of-pocket maximum of $8,550/$17,100 (Individual/Family).

EASE Bronze

EASE Bronze offers full coverage for covered preventive health services. It also provides a copay arrangement for many in-network providers, including limited in-network hospital services, office visits and other medical services. Formulary-restricted pharmacy benefits are also covered, depending on the type and tier of the drugs. There is no deductible, and there is an out-of-pocket maximum of $8,550/$17,100 (Individual/Family).

EASE Silver

EASE Silver offers full coverage for covered preventive health services. It also provides a copay arrangement for many in-network providers, including limited in-network hospital services, office visits and other medical services. Formulary-restricted pharmacy benefits are also covered, depending on the type and tier of the drugs. There is no deductible, and there is an out-of-pocket maximum of $5,000/$10,000 (Individual/Family).
