You have a choice of three national medical plan options, all administered by UnitedHealthcare: the Choice Plus PPO, HSA Choice Plus and Surest Plan. All three plans provide comprehensive coverage including benefits for prescription drugs, 100% coverage for in-network preventive care and convenient access to virtual visits. You can choose to see any doctor or other healthcare professional or facility (a provider) of your choice under all three plans.

UHC Elite Advocates
Sometimes, navigating the healthcare system can be confusing, time consuming and stressful. To provide you with an enhanced level of Advocate service, the UHC Elite Advocate team will be supporting employees and their covered dependents. The Elite care team includes advocates and dedicated nurses who can collaborate with other areas within UHC to provide you with support on care, claims and billing. You’ll be assigned a single point of contact to assist you from your first call to resolution.

Virtual Visits
When you need medical care your fastest and most cost effective option may be through a virtual consultation. Through UHC’s Virtual Visits model, you will have access to 24/7 care and have the flexibility to see a doctor without an appointment or the hassle of leaving your house. To make it easier for you to access virtual care, there is no cost for medical and behavioral health virtual care visits under the PPO plan, and after you meet your deductible under the HSA plan. (Dermatology virtual visits will remain at cost). UHC partners with Doctor on Demand, AmWell and Optum Virtual Care for virtual care services. You can initiate a virtual visit from the myuhc.com website or through the UHC mobile app.
2nd.MD
Through 2nd.MD, world-class providers in their area of specialty will provide you a no-cost second opinion on the care you’re receiving for musculoskeletal, cardiac, women’s health, oncology and gastrointestinal conditions. Specialists come from prestigious health systems like Harvard Medical School, Boston Children’s Hospital and UTHealth (Texas).
Maternity Support Program
If you or a covered family member may be thinking about having a baby or have one on the way, UHC’s Maternity Support Program will provide support during and after the pregnancy. You have access to an experienced maternity nurse who will provide assistance and guidance, including if you have unique health needs and/or a high-risk pregnancy. The Maternity Support Program also includes a comprehensive pregnancy app that provides personalized content and allows you to connect in real time with a nurse 24/7. You'll also receive a special maternity kit with helpful items to guide you through pregnancy and beyond.
Quit For Life® Smoking Cessation Program
UHC’s Quit For Life® program is a clinically proven smoking cessation program that’s offered in collaboration with the American Cancer Society. You and your covered family members have access to a Quit Coach whom you can work with to create a plan that’s personalized and tailored to your needs. Working with the Quit Coach, you’ll set a quit date and receive ongoing support, including text messages and access to online resources.
Health and Wellness Portal – Rally®
You and your covered family members (age 13 and older) also have access to UHC’s health and wellness portal called Rally®. Rally is designed to help you make changes to your daily routine, set smart goals and track your progress. You’ll get personalized recommendations to help you move more, eat better and improve your health — and have fun doing it. You can access Rally from the myuhc.com website or the Rally mobile app.
Cancer Resources Services
Through UHC’s Cancer Resources Services, you and covered family members will get personal support from an experienced cancer nurse. They can help answer questions about a cancer diagnosis and share information to help you make informed decisions about cancer treatment.
Complex Care Concierge
UHC’s Complex Care Concierge proactively identifies employees and family members covered under Santander’s medical plans who have complex care needs. The Concierge reaches out and provides you with a dedicated care advisor. Your care advisor is responsible for optimizing your benefits, and ensuring your care is cohesive and connected.
Fertility Solutions
If you or a covered family member is considering fertility treatments, you have access to the UHC Fertility Solutions Program. Through this program, you’ll work with UHC specially trained fertility nurses who will provide you with clinically guided support and education, and help you identify UHC Centers of Excellence (COE) for fertility treatment(s). Both plans cover in-network medically necessary fertility treatment(s) at a COE, up to a $35,000 combined lifetime maximum for medical and prescription drugs.

Neonatal Resources
We want to make sure our babies and new mothers receive the best care possible. If you have a baby born before full term or with healthcare issues, your medical plan coverage includes specialized clinicians who collaborate with your provider and work directly with you to provide the best care. This includes early interventions to ensure appropriate care before delivery, and post-discharge support for you and your baby for up to one year.
Specialist Management Solutions
If you and your doctors determine that you or a family member needs outpatient surgery, UHC’s Specialist Management Solutions (SMS) team can help you understand your options, identify the right facility, facilitate scheduling, and communicate with providers. You may be proactively contacted by an SMS specialist if you or your family member qualifies as part of the program.
Bariatric Resource Services
If you or a covered family member is considering weight-loss surgery, you have access to the UHC Bariatric Resource Services Program. Through this program, you’ll work with a UHC case manager who will provide you with education and personal support, and help you identify UHC Centers of Excellence (COE) for bariatric surgery. Both plans will cover in-network medically necessary bariatric surgery at a COE, one per lifetime.

Mental/emotional health is just as important as physical health. When dealing with a chronic health condition it’s easy to feel overwhelmed or discouraged. You have access to a confidential virtual therapy program called AbleTo to work with a licensed therapist to work through your health challenges. Counseling sessions with a licensed therapist are 1-on-1 through phone or video chat. UHC partners with AbleTo to provide this program.

Learn cognitive behavioral therapy (CBT) techniques, access on-demand coping and meditation tools, and find support in a vibrant peer community. You can download the app from myuhc.com.


Coverage Outside the United States
If you travel outside the United States, coverage is available under both plans, although you typically will pay for services out-of-pocket and then submit requests for reimbursement using the International Claims Transmittal form, which is available on the myuhc.com website.
- Emergency services outside the U.S. are covered at the in-network level based on billed charges and are subject to the annual deductible.
- Non-emergency services are covered at the out-of-network level based on billed charges and are subject to the annual deductible.

Real Appeal- Weight Loss Support
Real Appeal is an online health program designed to help you achieve long-lasting results by learning how to manage your weight, increase your physical activity, and eat more nutritiously. This program is available to you and your covered family members age 18 and older with a Body Mass Index of 23 or higher. You'll be connected to a weight loss coach and encouraged to attend online coaching sessions. At the start of the program you'll receive a success kit in the mail that includes a cookbook, weight scale, food scale, exercise bands, and more.
