Enrollment Window
New Hires: You have 30 days from your hire date to enroll in benefits. If you do not enroll, you will not be able to choose benefits until our next Open Enrollment.
Medical and Prescription Changes
The 80 and 80HD plans have deductible, out-of-pocket maximum, and co-pay changes. The Wrap Plan has new eligibility criteria. SwedishAmerican associates whose primary insurance is with SwedishAmerican (under either the 80 Plan or the 80HD Plan) will not be eligible to elect the Wrap Plan. Prescription plan co-pays are changing for both the SwedishAmerican retail pharmacy and the Elixir contracted pharmacies.
Short-Term Disability Plan Changes
SwedishAmerican offers a 60% short-term disability benefit plan. The 75% disability has been eliminated. All other disability plan provisions will remain unchanged. NOTE: There is no guarantee of disability benefits. Disability benefits are subject to plan provisions in effect at the time a claim is submitted.
