Anthem EPO Medical Plan: Prudent Rx
The Anthem EPO medical plan includes PrudentRx, a prescription support program through CVS Caremark designed to help you save on specialty medications. Participating members can enroll in the program to receive $0 out-of-pocket cost on eligible specialty medications.
Opt-out Credit
If you're covered under another family member’s medical plan, you can waive TK Elevator Medical Plan coverage and receive a $1,000 annual Opt-Out Credit. Be sure to compare plan benefits, costs, and contributions before choosing to opt out. To qualify, elect or re-elect this option during enrollment and submit verification of other coverage (such as an ID card or application from the other plan). The Opt-Out Credit activates upon proof of other coverage.
You won't receive credit if enrolled in a state or federal high-risk pool. Dependents of TK Elevator employees under their coverage (excluding NEI Benefit Plan) aren't eligible to receive the Opt-Out Credit. Coverage will not continue unless you actively re-enroll.
Wellness Program
Our Wellness Program through Personify Health was discontinued as of December 31, 2025.
Please note the following:
- The last day of coaching was December 31, 2025.
- The last day to redeem earned points is January 30, 2026.
- After January 30, 2026, any unused points will expire and will no longer be available. For questions, please email support@personifyhealth.com.
We encourage all participants to log into their Personify Health accounts to review and redeem any outstanding rewards before the deadline.
