Weight Management

AEP covers the cost of this confidential program for AEP Medical Plan participants and covered family members age 18 and older.

Some qualifying risk factors include Type 2 diabetes, obesity, high blood pressure and high cholesterol.

Losing 5 to 7 percent of your body weight actually lowers your risk of many chronic diseases and help to keep them in-check. In fact, that’s the goal of this nationally recognized lifestyle change program.

You’ll take a one-minute quiz asking about your risk factors to help you understand if this program can help you. Then, Solera Health will help you decide whether an in-person support group or an online option is the best fit for you.

Most programs include access to a personal health coach, a small group for support, and tools like a scale with wireless internet connectivity. At week 4 of the program all participants will receive a FitBit activity tracker.

Diabetes Prevention Program

AEP offers a benefit at no cost to help Medical Plan participants and their families live healthier lives—a diabetes prevention program that offers in-person group meetings and online options for individuals at risk for developing type 2 diabetes.

The program, offered by Solera Health, in partnership with AEP and Anthem, helps you make lasting changes by eating healthier, increasing physical activity, and managing the challenges that come with lifestyle change. The result: You'll feel better and reduce your risk of developing type 2 diabetes.

And, you'll receive a free FitBit activity tracker!

How to Get Started With Solera Health

Go to solera4me.com/aep and take a 1-minute quiz. Any personal information you provide as part of the quiz will be treated confidentially and will not be shared with AEP. You can also call Solera Health to take your quiz if you would prefer.

At the end of the quiz, you will be told whether or not you qualify. With just a few more questions, Solera Health will help you decide whether an in-person support group or an online option is the best fit for you. Then you will be enrolled in a program and ready to get started!

Most programs include access to a personal health coach who facilitates the weekly lessons, a small group for support, and online support and resources. Also available are tools like a scale with wireless internet connectivity. At week 4 of the program, all participants will receive a FitBit activity tracker.
