What Happens to Coverages Due to a Qualified Life Event or Change in Work Situation

You can change Optional coverages under the Flexible Benefits Program outside of Annual Enrolment only if you experience a qualified life event. And, under certain work situations coverages continue while other coverages end.

WHAT’S A QUALIFIED LIFE EVENT

A change in your personal situation that allows you to change your existing Optional coverage elections under the Flexible Benefits Program. Any change to coverage must be consistent with your qualified life event.

EXAMPLES OF QUALIFIED LIFE EVENTS

Examples of qualified life events may include:

  • Marriage or a common-law union (common-law union requires one year of cohabitation)
  • Birth or adoption of a child
  • Legal separation or divorce
  • A dependent’s change in eligibility
  • Change in coverage under another program
  • Your spouse’s change in employment
  • Death of a spouse or child

Planning to retire?

Upon retirement, you may be eligible for certain retiree coverages and retirement benefits through MDLZ. Before you start the retirement process, learn what may be available to you.

MAKING CHANGES TO YOUR COVERAGES

If you experience a qualified life event and as a result want to make a change to your Optional coverages under the Flexible Benefits Program, contact the Mondelēz Canada Benefits Centre at 1-800-641-1500 (Option 1 – Benefits; Option 1 – Health & Welfare Benefits) within 31 days of the date of the event. The change takes effect retroactive to the date of the event. If you don’t notify within the 31-day period, you’ll need to wait until the next Annual Enrolment period to make any changes to your Optional coverages.

Note: If evidence of good health is required, the change only takes effect once the carrier—Sun Life and/or Sutton—approves the evidence of good health. And, if you’re not actively working when the change occurs or when Sun Life and/or Sutton approves the evidence of good health, the change only takes effect once you return to active work.

WHEN COVERAGES END

Coverages generally end once you (or your covered dependent) no longer meet the Flexible Benefits Program’s eligibility requirements. Coverages end if you:

  • Terminate from MDLZ;
  • Retire from MDLZ (unless you’re eligible for certain retiree coverages in place on your retirement date); or
  • Die (Health and Dental coverages continue for your covered dependents through the end of the third month following your date of death).

Core Life Insurance and Optional Life Insurance coverages end on the date you terminate or retire. You (or your spouse) may convert coverage to an individual policy at your own cost (you must apply for a conversion within 31 days).

If you die, your designated beneficiary(ies) receive a death benefit based on the following coverages in effect on the date of your death:

  • Core Life Insurance;
  • Optional Life Insurance (if applicable);
  • Optional AD&D Insurance (if applicable) if your death results from an accident; and/or
  • BTA Insurance coverage (if your death results from an accident while traveling on Company business).

That’s why it’s important to keep your beneficiary information on file with the Mondelēz Canada Benefits Centre up to date. See Life, Accidental Death and Dismemberment (AD&D), and Business Travel Accident (BTA) Insurance for additional details about naming your beneficiary. Your covered spouse (if applicable)—in the event of your death—may convert their Optional Life Insurance coverage to an individual policy by applying for a conversion within 31 days.
