How to make your elections

FLEXIBLE BENEFITS PROGRAM

Mondelēz Canada provides you with an online enrolment tool for the Flexible Benefits Program called MyBenefits Online. This tool is designed to make enrolment as easy as possible as a new hire and each year during Annual Enrolment.

You can access MyBenefits Online 24/7. This tool enables you to try different coverage scenarios, apply your allocated Flex Credits to the Optional coverages you elect, and instantly see the cost impact associated with each scenario. If you’re new to MyBenefits Online, here’s how to access and register your account.

  • Connect directly by going to https://mondelezcanada.hroffice.com
  • Follow the prompts to register.
  • Once registered, you’ll simply enter your Mondelēz Canada Employee ID and the password you set as a first-time user (you can find your Employee ID number in Workday as well as on your paycheque).

Once you have a registered account, you can continue to access MyBenefits Online directly, or use the single sign-on (SSO) feature available through the Employee Center. Here’s how:

  • Log in to your MDLZ-provided computer (if you’re an hourly employee, use the on-site kiosk) with your Lan ID and established password.
  • Access the Employee Center.
  • Select the MyBenefits Online Canada tile under the My Applications section of the Employee Center’s home page.

Once you’re logged in, select Restart or Modify on the New Hire/Rehire event that’s displayed at the top of the home page, and follow the prompts to enrol (if you’re making coverage elections).

Important note about the flexible benefits program

It’s your responsibility to familiarize yourself with the Flexible Benefits Program’s eligibility and enrolment requirements.

If you don’t enrol within the 31-day enrolment event, you won’t have Health or Dental coverage for your eligible dependents for the remainder of the current year. You’ll receive default Core and Optional coverages for you only, and you’ll need to wait until Annual Enrolment to elect Optional coverages for yourself and your eligible dependents for the following year (unless you experience a qualified life event).

Additionally, if you don't enrol within the 31-day period and name a beneficiary for your Core Life Insurance coverage, a death benefit will be paid to your estate in the event of your death.

KEY FEATURES OF MYBENEFITS ONLINE

Here’s what you can do on MyBenefits Online:

  • Watch the video tour: It walks you through how to navigate the portal and complete your enrolment online. The video is located on the home page.
  • Access tools and resources: They’re there to help you make coverage decisions. Be sure to also reference the more detailed coverage information in the Resource Library.
  • Update your dependents: Add or change who you’d like to cover.
  • Review, change, or add beneficiaries: Ensure your beneficiary designation is correct and up to date for all applicable coverages under the Flexible Benefits Program.
  • Manage your Personal Profile: This tile gives you access to your Current Coverage page and various other tabs where you can view your designated beneficiaries and personal information about you and your dependents.
  • Know your coverages: Review the coverages you have in effect right now (and for whom). And, access the site’s Resource Library for more detailed materials provided by the carriers/vendor partners.
  • Know your costs: Review the Flex Credits that are available to you to help offset the cost of the Optional coverages you elect. Remember, as an eligible employee the Company provides you with $400 in Flex Credits. And, if you’re a full-time employee and you remain at or elect the 50% Core LTD option instead of the 60% Core LTD option, you receive an additional $400 in Flex Credits (for a total of $800), as shown below.
  • Get your questions answered: Complete the form under the site’s Ask a Question area, and a Mondelēz Canada Benefits Centre representative will provide a response within two business days.

QUESTIONS, OR TO ELECT OR CHANGE COVERAGES BY PHONE

If you’re not comfortable using the MyBenefits Online tool, you have additional questions, or you want to elect/change your coverages by phone, call the Mondelēz Canada Benefits Centre at 1-800-641-1500 (Option 1 – Benefits; Option 1 – Health & Welfare Benefits). Representatives are available Monday through Friday, 8:30 a.m. to 5:00 p.m. (Eastern Time).

ENROLLING WHEN NEWLY ELIGIBLE

You’re automatically enrolled for certain Core coverages under the Flexible Benefits Program. However, you must take action if you want to enrol for Optional coverages—especially if you want to elect Family coverage for your Health and/or Dental coverage (otherwise your eligible dependents won’t be covered).

You have a 31-day enrolment window to enrol. If you don’t, you receive the following default coverages:

  • Basic Health (single coverage category)
  • Basic Dental (single coverage category)
  • Core Life Insurance (if you don't name a beneficiary, a death benefit will be paid to your estate in the event of your death)
  • Short Term Disability (STD)
  • 60% Core Long Term Disability (LTD)—full-time employees only
  • Business Travel Accident (BTA) Insurance
  • Employee Assistance Program (EAP)—you and your household family members can use the EAP’s services

Default coverages take effect on your date of hire and continue until December 31 of the year in which you’re hired. You can then choose new Optional coverages and/or coverage categories during Annual Enrolment for the following year, or earlier if you experience a qualified life event.

ENROLLING DURING ANNUAL ENROLMENT

Each Annual Enrolment, you can review your current coverages against the following year’s coverage options, their costs, and the Flex Credits you’ll have available to help pay for the Optional coverages you elect. Depending on your personal situation, you can do any of the following relative to your Optional coverages:

  • Make any necessary coverage changes for the following year;
  • Maintain the same coverages—at the Optional coverage’s new cost (if applicable); or
  • Review and/or update your designated beneficiary(ies); or
  • Opt out of coverage(s).

Note that if you opt out of any Optional coverage, you must wait until the next Annual Enrolment (or earlier if you experience a qualified life event) to enrol. The elections you make during Annual Enrolment take effect the following January 1.

It’s important to review your Optional coverage elections each year to ensure they’ll best support the well-being of you and your family. Annual Enrolment is also a good time to make sure your dependent and beneficiary information is up to date for all applicable coverages.

If you don’t complete your enrolment or make any changes by the Annual Enrolment deadline, the Core and Optional coverages in place at the end of the year carry over to the following year (at the Optional coverage’s cost for the following year). If for some reason your coverages won’t carry over to the following year, you’ll be notified so that you can take action accordingly.

COORDINATION OF COVERAGES

You and/or your covered dependent(s) may have health and/or dental coverage under a program outside of MDLZ. If this is the case, you can coordinate the Flexible Benefit Program's benefits with your spouse's program's benefits. This enables you to maximize coverage when your (or your covered dependent’s) eligible expenses are entitled to benefits from each. Here’s what to do in these instances.

  • As the covered employee, you and your covered spouse should first submit the eligible expense to your respective programs. You can then submit the eligible expense to your spouse’s program (and vice versa) for the portion of the eligible expense that isn’t paid for by your own programs.
  • For any covered child’s eligible expense, submit the dependent’s eligible expense to the program of the parent whose birthday (month and day; the year isn’t important) is earlier in the year. So, if your birthday is in November and your spouse was born in March, submit claims for your covered dependent’s eligible expenses to your spouse’s program first.
  • If, after submitting expenses to your and your spouse’s programs you still have out-of-pocket expenses that remain, submit them to your Health Care Spending Account (HCSA) for reimbursement (if you have an HCSA balance). Please see the GreenShield Benefits Booklet for information or contact GreenShield directly for assistance.

And, if your spouse is also an employee of MDLZ, the Flexible Benefits Program coordinates benefits for your and your spouse’s eligible expenses.

When you can change your elections

During the year, you can change most Optional coverage elections only if you experience a qualified life event. See Life Changes for details regarding qualified life events.
  • You may make changes to Optional coverages under the Flexible Benefits Program each year during Annual Enrolment. The elections you make during the Annual Enrolment period take effect the following January 1.
  • You may elect or change your Optional Life Insurance coverage if you experience a qualified life event (or during Annual Enrolment), though evidence of good health may be required. See Life for details regarding evidence of good health.
