What is dependent verification?

After you enroll new dependents, you will be asked to provide certain paperwork to verify their eligibility. It will include items such as a marriage license, proof of current financial responsibility, birth certificate, etc. It’s important to know forging information has consequences, including cancellation of dependent coverage and disciplinary action that could include termination.

When do I need to complete dependent verification?

Documentation for dependent verification must be provided within 30 days. If you are adding a dependent due to a qualifying life event, changes must be requested within 31 days of the event.

NOTE: Changes won’t be allowed if you miss these deadlines.

Why do I need to verify my dependents?

To ensure all dependents added to your benefits are eligible for coverage, we need to verify their relationship to you. Providing proof of your relationship through certain documentation, known as dependent verification, ensures anyone you have added to your plan(s) can be covered.

Dependent Verification Checklist

  • Look for the notification: Once you have finished enrolling in your benefits, look out for a notification indicating that documents need to be uploaded for dependent verification on your enrollment site account or on the MyChoice Mobile App. You will also receive email and text notifications if you’ve opted in.
  • Submit documentation by:
  • Snapping a picture in the MyChoice Mobile App: Open your messages and follow the prompts to upload a picture.
  • Scanning and uploading by logging in through hertzbenefits.com: Visit your Message Center and reply to the recent Submit Documentation to Verify Eligibility message. Upload your document(s) directly to the message (PDF or JPEG file types are recommended).
  • Email: If you need to email your documents, follow these instructions.
  1. Please reference the Verification Initial Letter in your Personal Documents to see what documents are required. You will need to download the document so you can include the cover letter included in that document.
  2. In the subject line, please include Hertz and your name, such as: Hertz – John Smith
  3. Include as attachments:
  • The cover letter from your Verification Initial Letter
  • Your supporting document(s)
  • Any supporting details
  • Please send documents and supporting details as attachments and not as text in the body of the email.
  • Look out for confirmation: If you are uploading documents directly to the site, please allow 5-7 business days to process. If you are mailing documents in for verification, please plan for 7-10 business days for the documents to get to us, and then allow 5-7 business days to process.
